CLINICAL CASE: # 3

Carrie Williams has been admitted to the Psychiatric Unit at Bayshores Treatment Facility. She is eight-and-one-half months pregnant and in a manic state. She has not slept for three days, is hyperactive and hypervoluble. When you come on duty, she is running all over the Day Room, jumping from chairs and standing on tables saying to everyone: “I am Cicely Tyson and I will perform for you. My agent wants you to listen to me and come to see my movies. I’m going to Hollywood and if you treat me right I will take you along.”

Along with several other nurses, you decide the patient must be placed in seclusion for her protection and the safety of the fetus. Also, she is disrupting the Unit and upsetting the other patients. Tension in the Day Room is very high as Mrs. Williams climbs over the back of a sofa and almost falls over a table.

After being placed in seclusion, the patient rolls on the floor and bounces herself off the walls with her protruding stomach. The psychiatrist is notified and orders that liquid Haldol (an appropriate dosage for the patient) be injected into the patient’s sealed juice containers and that she be encouraged to drink the juice. As the nurse working with this patient what would you do?

Client states, “I recently lost my job and feel hopeless. I can’t sleep and don’t feel like eating.” Client also reports she has lost 10 pounds during the last two months. Client states that she is a solo parent and is worried about becoming homeless. Client states, “I worry all the time. I can’t get my brain to shut off. My husband is in the military and currently serving in an overseas combat zone for the next eight months. I worry about him all the time.”

Behavioral Observations

Client arrived 30 minutes early for her appointment. Client stated that she had never been in counseling before. Client depressed and anxious, as evidenced by shaking hands and tearfulness as she filled out her intake paperwork. Ana made little eye contact as she described what brought her into treatment. Client speech was halting. Client affect flat. Client appeared willing to commit to eight sessions of treatment authorized by her insurance company.

General Background

Client is a 24-year-old first-generation immigrant from Guatemala. Ana was furloughed from her job as a loan officer at local bank three months ago. Client reported that she was from a wealthy family in Guatemala, but does not want to ask for help. Client speaks fluent Spanish.

Education

Client has completed one year of college with a major in business. Client states that she left college after her son was born as she found it difficult to manage a baby, college, and a full-time job.

Family Background

Client is the middle of four siblings. Client has two older brothers and one younger sister. Client’s parents have been married for 27 years. Client states that she has had a “close” relationship with her family, although she states that her father is a “heavy drinker.” Client states that all her brothers and sisters have graduated from college and have professional careers. Client states that her father is a banker and her mother is an educator. Client states that she has not seen her family for 1 year. Client has a 1-year-old son and states that she is sometimes “overwhelmed” by raising him alone.

I think most of us will agree out healthcare system is in need of a make-over, be it access to care, quality of care or the payment system. I can attest that the “HOW” has been a battle for the past 15 years and will likely continue to be debated for years to come.

As you may recall about a year ago the backlash Epipen received for inflating the product cost to exceed over 600% the value to develop the product. The cost of pharmaceutical drugs continue to increase as a disproportionate rate to the value of an individual’s cost of living. It’s unfair that in the United States persons have to choose between the medication and other essential hierarchy of needs. I encourage you to read the article below and consider your role as a financial manager. What recommendations would you provide to the stakeholders, R&D team and vendor/suppliers? Throughout this course, you will be designing a Counseling Group from start to finish. The assignment will be broken into four parts, which are due at different intervals in the course. Application: Developing a Health Advocacy Campaign

To be an effective advocate and to develop a successful health advocacy campaign, you must have a clear idea of the goals of your campaign program and be able to communicate those goals to others. In addition, it is the nature of nurses to want to help, but it is important to make sure that the vision you develop is manageable in size and scope. By researching what others have done, you will better appreciate what can realistically be accomplished. It is also wise to determine if others have similar goals and to work with these people to form strategic partnerships. If you begin your planning with a strong idea of your resources, assets, and capabilities, you will be much more likely to succeed and truly make a difference with those you hope to help.

 

You will develop a 10- to 12-page paper that outlines a health advocacy campaign designed to promote policies to improve the health of a population of your choice. This week, you will establish the framework for your campaign by identifying a population health concern of interest to you. You will then provide an overview of how you would approach advocating for this issue.

 

-In Week 9, you will consider legal and regulatory factors that have an impact on the issue and finally, in Week 10, you will identify ethical concerns that you could face as an advocate. Specific details for each aspect of this paper are provided each week.

 

This paper will serve as the Portfolio Application for the course.
